Social Resource Navigator Program

When health care providers and community-based organizations (CBOs) work together, they can address the factors that shape your health – things like having enough food, a safe place to live, reliable transportation, education and employment opportunities, and interpersonal safety with family and friends.

The Social Resource Navigator (SRN) Program is a collaboration between Cornell Cooperative Extension Tompkins County, Cayuga Health, and Cornell University. The Navigators work with community members to facilitate connections to resources that can meet their health-related social needs like the ones listed above.

Here’s how the program works:

  • If you are a Cayuga Health patient, you will be offered an optional social needs screening form when you visit your primary care provider’s office.
  • If you share any concerns, you will get a call or text from a Resource Navigator.
  • The Resource Navigator will listen and work with you to connect to resources that are the best fit for you. They will follow up to see how things are going.
  • You choose what to share about yourself. You can say yes or no to any resources offered.

Resource Navigation is free and for everyone.
